"Medical Home" refers to a continuous relationship between a personal physician and a patient enhanced by a comprehensive, integrated approach to the practice of healthcare.

According to a 2007 joint statement by the American Academy of Family Physicians, American Academy of Pediatrics, American College of Physicians, and American Osteopathic Association—the leading primary care physician organizations, the concept of the medical home refers to an integrated model of healthcare delivery that features a commitment to coordinated, continuous, comprehensive, accessible and culturally sensitive healthcare in addition to centralized medical record keeping.
